We ask the Mentor Teacher to provide us with two official evaluations, the Mid-Semester Summary completed at mid-semester and the End-of-Semester Evaluation at the end of the semester. The forms vary slightly and both should be completed online. Completing these evaluations offers the Mentor Teacher an opportunity to sit down with the student teacher and possibly the university coach to provide both formative and summative feedback.

Web-based versions of these forms are included in your Student Teaching binder in Watermark-TK20 (https://edtemple.tk20.com). You can download/print copies of the forms for your reference here, but all observation forms should be completed and submitted online via Watermark-TK20.
